Student Cards

Please Note: there is a $10 fee to replace lost student cards

The library is excited to announce our new on-line student card request system.

Cards with student photographs can be picked up in person at Campus Libraries.
If you also need a clinical badge, please use the "Pick up in Person" form.

Cards without a photograph can be mailed to distance learning students and must be used in conjunction with another form of photo ID.
